An amazing new job opportunity has arisen for a committed, enthusiastic and dynamic Advanced Nurse Practitioner in our Acute Response Team in Cardiff and Vale UHB. The post holder will be able to demonstrate advanced and specialist nursing practice in holistic assessment, planning, implementation and evaluation of care for patients and their families in the Community.

This will necessitate high levels of autonomy and advocacy e.g. Nurse Led Clinics, undertake clinical community visits, receiving direct referrals, following appropriate training / clinical supervision. Working within the agreed Policies and procedures of the Cardiff and Vale University Health Board and to work within the Professional Code of Conduct of the Nursing and Midwifery Council.

The role of the Advanced Nurse Practitioner will be to lead the Acute Response Team Clinical to develop and grow the service to meet the demands of the population. The post holder will assess own learning needs through the process of performance review and develops key annual objectives which support the delivery and improvement of the service. Develop and enhance clinical and leadership skills as appropriate, undertaking in-house and formal training to ensure competency in all aspects of care and participate on a national scale to represent the service. Be aware of all aspects of clinical governance and risk management with regards to the enhanced role of the nurse practitioner.

Maintain clinical credibility by demonstrating continual professional development and undertake training and education as required, ensuring highest standard of evidence based clinical skills are demonstrated.

Undertake teaching sessions and presentations to groups of staff; be involved with the training and education of other staff with regards to the role of the UCC and ENP/ANP's.

To adapt and develop the role of Nurse Practitioner, ensuring flexibility and willingness to expand the role as required and to lead and develop the Acute Response Service in Cardiff and Vale.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The Acute Response Team is a pan-locality service which supports all residents within the Cardiff and Vale of Glamorgan area. The population is dynamic and varied with a wide range of geographical areas covered.
